Medical
Employees can choose coverage for themselves and their eligible dependents through two Personal Health Account Plans, Enhanced or Standard. Each offer a health savings account. The Enhanced plan includes an employer subsidy, where the Standard plan does not. Retail and mail order prescription drug coverage is available as part of the plans. Under the Health Savings Plan, you may contribute up to a total of $3,250 for individual coverage or $6,450 family coverage (which includes the company contribution, if applicable) for qualified medical, dental and vision care expenses. The premium is paid on a before-tax basis.
Dental
A comprehensive dental plan is available for employees and their eligible dependents. The plan covers diagnostic and preventive services, basic, major and orthodontic services. The premium is paid on a before-tax basis.
Vision
Coverage is available for employees and their eligible dependents. The plan includes eye exams with a modest co-pay, and discount frame, lens and contact lens coverage. The premium is paid on a before-tax basis.
Flexible Spending Accounts
There are two types of flexible spending accounts: Health Care FSA and Dependent Care. Each allows you to set aside a portion of your earnings to pay for either qualified medical, dental, vision or dependent care expenses on a pre-tax basis. You may contribute up to $2,500 annually for each account. However, if you participate in a Health Savings Account, the Health Care FSA is limited to dental and vision expenses and cannot be used for medical expenses.
401(K) Plan
You are immediately eligible to participate in our 401(k) Savings Plan. You can contribute from one to 50% of your salary on a before-tax basis, up to the IRS allowable limits. GDATP matches 50% up to the first 7%. You are vested in the company match after completing three years of service.
